For this functionality, there’s nothing better so far than Rob Marsh’s “Similar Posts” plugin. Very light and well done code, and it comes with an optional output cache if you wish to use it.

An improvement to the full-text search that allows limiting by a relevance threshold allows the most relevant posts to show up at the top of the list, so if you only display 5 results, they’re more likely to be of interest.
